The December 12th MSQC Collaborative-Wide Meeting will bring together surgeons, nurses and other members of the healthcare team from across the state of Michigan to discuss data, identify trends and hear from renowned speakers on a variety of topics that will address knowledge gaps applicable to all members of the healthcare team.

Implementing Collaborative Care (“Base Training”) is designed for Patient-Centered Medical Home-designated practices that have completed the Collaborative Care Model (CoCM) practice assessment with a determination that the practice is ready for implementation.
