APA Objectives
  1. Discuss the PACE (Program of All-Inclusive Care of the Elderly) model of care, including eligibility, funding, and novel services provided to enrolled Medicare/Medicaid dual-eligible elderly allowing them to remain in the community, rather than being placed in a state-funded nursing home bed.
  2. Describe the process of establishing an integrated psychiatric and behavioral health service at the local Huron Valley PACE, including the role of geriatric psychiatry services addressing later life psychiatric illnesses and dementia, the interface with PACE Primary Care, the PACE behavioral health team, and other members of the PACE interdisciplinary team (IDT).
