Overview
Implementing Collaborative Care (“Base Training”) is designed for Patient-Centered Medical Home-designated practices that have completed the Collaborative Care Model (CoCM) practice assessment with a determination that the practice is ready for implementation. This training is intended to help the primary care physicians, psychiatric consultants, behavioral health care managers, and other primary care practice team members gain the foundational knowledge and skills for implementing CoCM.

Objectives
At the conclusion of this activity, learners will be able to:

  1. incorporate workflows and other operational techniques unique to Collaborative Care within their practice to address the behavioral health needs of patients with fidelity to the Collaborative Care Model. They will be able to implement the Collaborative Care Model in their practices to address the behavioral health needs of the patient population.
  2. Describe the role of each care team member in Collaborative Care.
  3. List the five principles of Collaborative Care.
  4. Discuss the rationale for Collaborative Care and the evidence supporting its use.
  5. Describe which patients are appropriate for Collaborative Care.
  6. Select screening and ongoing monitoring tools to use in Collaborative Care.
  7. List the required and recommended features of a systematic case review (SCR) tool.
  8. Prioritize patients to be discussed during systematic case review (SCR).
  9. Define roles of additional team members involved in successful Collaborative Care implementation
  10. Discuss ways to measure performance of a Collaborative Care program.
  11. Describe how Collaborative Care billing codes differ from other types of billing codes.
  12. Discuss the key roles and responsibilities of the behavioral health care manager in Collaborative Care.
  13. List sources of information used to inform patient assessment.
  14. Discuss the fundamental engaging skills of OARS (Open-ended questions, affirmations, reflections, and summaries)
  15. Explain the rationale for behavioral activation.
  16. Discuss what to include in a self-management plan.
  17. Recognize common psychotropics used to manage depression and anxiety.
  18. List the essential information to include during a systematic case review presentation.
  19. List and describe the purpose of each section of the relapse prevention plan.
